About The Position

Father Bill’s & MainSpring is committed to ending and preventing homelessness in Southern Massachusetts with programs that provide emergency and permanent housing and help people obtain skills, jobs, housing, and services. We help people who are struggling with homelessness or are at risk of homelessness to achieve self-sufficiency. Father Bill’s & MainSpring is a growing and dynamic social service agency with particular expertise in emergency and permanent housing, job training and placement, and prevention services for individuals and families in need. Over the next five years, our strategic focus includes an increase in affordable, permanent supportive housing, a more efficient model of care for families experiencing homelessness, commitment to an enhanced delivery model to reduce the need for shelter, and improved community relations to achieve these goals. The Rapid Rehousing Stabilization Case Manager (RRSCM) provides immediate engagement upon referral from HRC exits and street outreach to ensure a warm handoff into stabilization services. The person in this position will work with clients to develop person-centered stabilization plans focused on housing retention, safety, income and benefit stability, tenancy skills, and preventing a return to homelessness. This role requires strong coordination with internal teams and external providers, including HRC staff, street outreach teams, Rapid Re-Housing providers, the front door diversion team, landlords, household members, and other community partners to support smooth transitions and continuity of care.

Requirements

  • 2 years of relevant experience and/or a Bachelor’s Degree in social work or related field.
  • CPR certification is required and provided by FBMS.
  • AED and First Aid may also be required and will be provided by FBMS.
  • Empathy: An understanding and empathy for those dealing with homelessness and related issues.
  • Professionalism: Maintains the highest level of professionalism in the delivery of services, building of relationships, collaboration with co-workers, and representation of Father Bill’s & MainSpring within the organization and the community.
  • Communication: Speaks appropriately in positive and/or negative conversations with supervisors, co-workers, volunteers, donors and constituents; Articulates the goals of Father Bill’s & MainSpring.
  • Record-Keeping: Routine clerical or record-keeping incidental to the essential responsibilities of the job (including, but not limited to, data entry and case file updates).

Responsibilities

  • Deliver short-term, light– to medium-intensity stabilization services to individuals exiting homelessness into permanent housing who do not have ongoing case management supports.
  • Screen for risk factors that drive housing loss (i.e., arrears, conflict with household members, lease compliance issues, safety concerns, lack of income/benefits, unmet health needs).
  • Identify early warning signs of destabilization and implement rapid problem-solving to address issues before they escalate.
  • Complete home visits and assist participants in accessing community resources, scheduling, and attending appointments (transportation may be required). Maintain accurate case records and collect information in a timely and organized manner.
  • Support applications and follow-up for benefits and income resources (e.g., SNAP, SSI/SSDI supports, MassHealth/insurance navigation, employment resources, etc.).
  • Refer and coordinate access to health, behavioral health, substance use, legal, domestic violence, and other supportive services as needed.
  • Provide coaching around tenancy readiness and stability (e.g., budgeting, lease compliance, communication with landlords/household members).
  • Assist with landlord/household mediation and problem-solving for issues such as guest policies and household agreements, noise/behavior complaints, building rule compliance, and repair requests.
  • Connect clients to housing retention and financial stabilization supports, prioritizing existing resources before flex funding (e.g., RAFT, utility assistance, food programs).
  • Provide structured follow-up (e.g., initial intensive engagement, then tapering contact as stability improves) and goal setting to ensure long-term tenancy sustainability.
