This role works with individuals and families in a Transitional Housing/Rapid Rehousing Program, focusing on three basic outcomes: Daily Living, Financial Stability, and Permanent Housing. The Case Manager will conduct initial assessments, develop and implement housing plans with participants to end their homelessness, and monitor their progress. They will also provide linkages and referrals to appropriate agencies, act as an advocate, and offer case management in various settings including the participant's home, the community, and the facility. The position involves completing daily, weekly, and monthly paperwork accurately and in a timely manner, developing discharge plans for permanent housing, and gathering statistical information for funding bodies. Additionally, the role includes attending community networking meetings, providing community education about homelessness and The Salvation Army, and participating in professional development workshops. The Case Manager will represent the agency in the community as assigned, provide On-Call coverage, and comply with all confidentiality laws. Other duties may be assigned by the Director.
