Requirements

• Two years college education or equivalent work experience
• Must be able to travel between clinic offices
• Excellent presentation, verbal and written communication skills, and ability to collaborate with co-workers, senior leadership, and other management.
• Proficient in word processing, spreadsheets (excel), database and typing

Nice To Haves

• 1-2 years of IPA/managed care experience preferred

Responsibilities

• Works closely with the Quality Manager, Medical Director and Administrator in implementing quality platforms, including chronic care management (CCM), transitional care management (TCM) and other quality initiatives including HEDIS, P4P, MIPS, STAR measures, and annual wellness visits.
• Assesses missing patient benchmarks and organizes the data in preparation for member outreach or encounter with healthcare provider.
• Through a combination of mailing patient due letters or education flyers, calling the patient and/or healthcare provider, and creating referrals, the QC is instrumental in patient satisfaction and quality outcomes.
• Provides general education related to the quality measures missing or “due” to healthcare provider.
• Reviews gaps in care reports and educates providers to close data gaps.
• Provides back up to the Member Services Representative.
• Performs other duties assigned by the Quality Manager.
