Coastal Health is currently seeking a motivated and detail-oriented Quality Coordinator to join our team. This role involves coordinating patient care, ensuring timely follow-ups, assisting patients in navigating care transitions, and acting as a liaison between patients, care teams, and families. The Quality Coordinator will also identify high-risk patients, facilitate access to case management programs, and ensure accurate documentation, coding, and billing. Additionally, the role includes providing medication education and promoting adherence. In terms of quality responsibilities, the coordinator will ensure all required forms are completed and submitted, serve as a point of contact for health screening events, and act as a resource for HEDIS initiatives. Collaboration with staff to improve workflows, documentation, and data abstraction is key, as is performing chart reviews for HEDIS and quality measures, conducting patient outreach, and coordinating MWOV outreach. The role also involves initiating dismissal paperwork for non-compliant patients. Other duties as assigned will also be part of the role.
