A Quality Care Coordinator (QCC) contributes to improved health outcomes for patients receiving services throughout the AHF Healthcare System through compassionate, proactive, collaborative care. The QCC may work in a hybrid arrangement involving remote HCC locations and field work. The role requires strong knowledge of Healthcare Effectiveness Data and Information Set (HEDIS) and HIV/AIDS Bureau Ryan White Standards. The QCC supports preventive health measure outcomes across all payor sources by addressing HEDIS and HIV/AIDS Bureau measures, gaps in care, retention, and barriers to care. Working collaboratively with AHF departments and entities, the QCC focuses on solutions and telephonic interaction with patients, advocating for key initiatives while keeping AHF’s Core Values and Mission central to their work. As part of a close-knit team supporting the Quality Care Management nursing team, the QCC conducts patient outreach, helps remove barriers, and facilitates access so patients can complete preventive health measures regardless of payor source. The position also follows up on transitions of care and/or chronic care management appointments. Patient retention is a priority; connecting with patients and providing compassionate care and empowerment are essential. AIDS Healthcare Foundation (AHF), founded in 1987, is the nation’s largest specialized provider of HIV/AIDS medical care. Its mission is to provide cutting-edge medicine and advocacy regardless of ability to pay. AHF describes its workplace as collaborative and dynamic, with staff accountable to multiple leaders and expected to adapt to changing needs. The organization’s core values are Patient-Centered, Value Employees, Respect for Diversity, Nimble, and Fight for What’s Right.
