This position performs outpatient behavioral health services to individuals across the entire LifePath system of care. The role involves maintaining a caseload and frequent travel to individual’s homes, community settings, nursing facilities, and hospitals within Collin County. Key responsibilities include ensuring individuals receive timely access to housing supports, collaborating with the LifePath Systems team and other community organizations to identify needs and reduce barriers to obtaining permanent housing. The position requires versatility in the SAMHSA Supportive Housing model, knowledge of community resources like Section 811 and Section 8, and effectiveness in troubleshooting and organizing support, interventions, and services to achieve outcomes. The role exemplifies service excellence through care coordination with a trauma-informed and person-centered approach, good stewardship of resources, and adherence to all relevant policies and regulations. It involves building positive relationships with community-facing agencies and maintaining professional conduct with peers, leadership, and support teams. The ability to assess and de-escalate behavioral health crisis situations calmly and respectfully is essential.
