About The Position

This position performs outpatient behavioral health services to individuals across the entire LifePath system of care. The role involves maintaining a caseload and frequent travel to individual’s homes, community settings, nursing facilities, and hospitals within Collin County. Key responsibilities include ensuring individuals receive timely access to housing supports, collaborating with the LifePath Systems team and other community organizations to identify needs and reduce barriers to obtaining permanent housing. The position requires versatility in the SAMHSA Supportive Housing model, knowledge of community resources like Section 811 and Section 8, and effectiveness in troubleshooting and organizing support, interventions, and services to achieve outcomes. The role exemplifies service excellence through care coordination with a trauma-informed and person-centered approach, good stewardship of resources, and adherence to all relevant policies and regulations. It involves building positive relationships with community-facing agencies and maintaining professional conduct with peers, leadership, and support teams. The ability to assess and de-escalate behavioral health crisis situations calmly and respectfully is essential.

Requirements

  • Bachelor’s degree in psychology, social work, or related field from a four-year college or university.
  • At least two years of experience in working with the community (i.e., social services, recovery, customer service, sales).
  • Reliable transportation and a valid driver license.
  • Knowledge and experience in providing case management, skills training, and symptom management services to individuals with mental illness.
  • Ability to assess individual needs, to coordinate services, and to communicate effectively.
  • Knowledge of the social services system and the ability to connect individuals with appropriate services.
  • Ability to write clearly and professionally.
  • Ability to think clearly and logically to deal with potentially dangerous and/or emotional situations.

Responsibilities

  • Perform outpatient behavioral health services to individuals in services across the entire LifePath system of care.
  • Maintain a caseload and frequently travel to individual’s homes, community settings, nursing facilities and hospitals within Collin County.
  • Ensure individuals receive timely access to housing supports.
  • Work with the entire LifePath Systems team and other community organizations to identify needs and reduce barriers to obtain permanent housing.
  • Be versatile in SAMHSA Supportive Housing model.
  • Navigate community resources including Section 811 and Section 8.
  • Troubleshoot and organize support, interventions, and services to achieve outcomes.
  • Provide care coordination with a trauma-informed and person-centered approach.
  • Ensure all required trainings are completed prior to due date and center-issued equipment is kept in good repair.
  • Ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, Substance Abuse and Mental Health Administration (SAMHSA) grant, other applicable grants, and Lifepath Systems policies and procedures.
  • Work closely with BH Division to ensure collaboration between the teams.
  • Build positive relationships with community-facing agencies and always maintain a professional relationship with all community stakeholders and partners.
  • Demonstrate professional conduct with Peers, Leadership and Support teams at all times.
  • Work independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals served and community partners and facilitating improvement in measurable ways.
  • Assess and de-escalate behavioral health crisis situations in a calm and respectful manner.
  • Provide and document engagement with individuals served and exceptional customer service in the electronic health record a minimum of half of time worked and meet UM standards by level of care.
  • Ensure 90% of service notes are signed within 2 business days of service.
  • Ensure 75% of full chart reviews receive a score of 80% or higher.
  • Understand and follow all rules for assisting with Section 811 and Section 8 applications as well as other public housing opportunities.
  • If Bilingual and receiving stipend, be available to translate for other staff as needed and utilize approved language in the provision of services as needed.
