This position reports directly to the Intensive Services Supervisor within the Behavioral Health division. This position will perform assessments, recovery planning, resource navigation, referrals to appropriate community services, supported housing, supported employment, and outpatient behavioral health services to include crisis services as needed, to adult individuals enrolled in services. This position provides flexible coverage and support for individuals receiving LOC 3 and LOC 4 services. This position may maintain a small caseload and is responsible for assisting with service delivery when staff members are on PTO, absent, or when additional coverage is needed. The QMHP-CS ensures continuity of care by providing skill-building, community-based support, crisis interventions, and rehabilitation services in accordance with individualized treatment plans and agency policies and procedures. This position requires adaptability, strong organizational skills, and the ability to effectively engage with a diverse population of individuals receiving behavioral health services. This position uses initiative and commitment to provide holistic care and works with the entire LifePath Systems team and other community organizations to identify needs and reduce barriers to improve social determinants of health. Must be effective at troubleshooting and organizing support, interventions, and services to achieve outcomes. Other duties may be assigned as needed. This position exemplifies service excellence by providing community-based psychosocial rehabilitation services and connects to other outpatient behavioral health services to include crisis services as needed. Services will be provided with a trauma-informed and person-centered approach. Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, applicable grants, and LifePath Systems policies and procedures. This position practices good stewardship of resources by ensuring all required training is completed prior to due date and center-issued equipment is maintained in good condition. This position builds and maintains positive relationships with mental health professionals and other community stakeholders. Must demonstrate professional conduct with Peers, Leadership and Support teams at all times. In addition, close working relationships and professional conduct while coordinating care with outside entities is required. Specific Targets: Engagement with individuals served and exceptional customer service will be provided and documented in the electronic health record a minimum 80 hours per month. Responsible for assessing need for client support funds and connection to the full array of behavioral health services. 90% of service notes are signed within 2 business days of service. Provides directed services to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services with a focus on promoting independent living skills in the community. All audit scores exceed 80% quality standard. Must understand LifePath Systems services and be able to explain to individuals served, family members, and community partners appropriately. If Bilingual and receiving stipend, employee will be available to translate for other staff as needed and will utilize approved language in the provision of services as needed.
