About The Position

This role involves working directly with individuals with developmental disabilities, ensuring person-centered approaches and agency VMP are incorporated into life strategies. The QIDP will assist in the development, implementation, and follow-through of individualized program plans, ensuring high-quality service plans that comply with all relevant regulations, agency policies, and procedures.

Requirements

  • At least one year experience working directly with people with developmental disabilities.
  • Must hold at least a bachelor's degree in sociology, social work, education, rehabilitation counseling, and/or psychology, OR be a doctor of medicine, OR be a registered nurse.
  • A current, valid driver's license is required.
  • Must meet the requirements related to Criminal History Record checks, which requires prospective employees to be fingerprinted if they have regular and substantial unsupervised or unrestricted physical contact with people receiving services in programs regulated by OPWDD.

Responsibilities

  • Ensures person-centered approaches and agency VMP are incorporated into life strategies as directed through CFAs, goals, Residential Habilitation Plans.
  • Assists in the development, implementation and follow-through of individualized person-centered program plans.
  • Ensures high-quality service plans are in accordance with all relevant regulations, agency policies and procedures and the Residential Standard Care Plan.
