About The Position

The Qualified Intellectual Disabilities Professional (QIDP) is responsible for leading the interdisciplinary team in the development and implementation of each individual’s plan of care; ensuring consistent service delivery of each plan, including but not limited to observing individuals, reviewing data and progress and revising programs based on individual need and performance.

Requirements

Bachelor’s Degree in Human Services or related field from an accredited university.
Minimum of two years’ experience working with persons with developmental and intellectual disabilities.
Must be able to read, write and speak in English.
Must have ability to manage, create, and lead programs.
Excellent administrative, clerical and organizational skills.
Computer literate especially working Microsoft Office and/or Google Suite)

Responsibilities

Functions as Circle of Support member to promote optimal resident/staff interactions.
Talks to residents in an appropriate manner.
Follows all Behavioral programs as written and assists with behavior management as needed.
Attends all behavior management meetings as required.
Models appropriate resident interactions.
Encourages all residents to be independent and participate in self-advocacy.
Provides direct care as needed/requested by house staff and supervisor.
Attends meetings as requested and gives feedback to house staff and supervisor.
Solves problems as necessary and communicates results or solutions to staff and supervisors.
Communicates with all staff to ensure consistency.
Facilitates communications between all disciplines through informal communications, meetings, and Individual Resident notes.
Keeps Residential Manager informed of unusual incidents or problems/concerns, which need interventions as well as commendations for staff as quality interactions are observed.
Responds to the needs and the preferences of the residents noting requests or reactions as they communicate them.
Facilitates the transition of residents to another house/facility as requested.
Ensures residents’ rights, wishes, desires, and individual needs are met through service delivery and programming.
Ensures Circle of Support meets annually and as needed with resident and/or their guardian and/or family to identify and meet residents’ needs, wishes, and desires.
Facilitates participation by members of the team to reach consensus on priority target behaviors/goals.
Participates in monitoring all behavior management programs and assure implementation and give input for revision, as needed.
Coordinates communication about monthly progress reports from all service areas and completes monthly report for each assigned resident.
Attends scheduled meetings (e.g., Behavior Management, Individual Resident meetings called by other disciplines) and ensuring follow through as needed.
Conducts Self-Advocacy meetings/forums and participates in advisory board meetings.
Acts as primary representative and advocate for individuals who attend public school.
Ensures that all residents’ individual programming service delivery and quality of life needs are implemented across all disciplines and service areas.
Ensures Skill Maintenance form has been completed for resident after goals have been met.
Ensures maintenance of skills is obtained by providing procedures for individuals after goals have been met.
Participates in gross motor activities as requested.
Ensures compliance with rules and regulations.
Maintains familiarity with State and Federal Regulations.
Ensures quality control of assessments, IP’s, implementation of program and services.
Participates in the survey process.
Assists with timely creation of Plans of Correction and performs follow-up as requested.
Monitors the implementation of corrective action and reports to the Program Manager (PM).
Ensures materials/ equipment is in place for programming needs.
Requisitions equipment and supplies, as needed, for programmatic purposes as requested by homes, PT, OT, or PT/OT Liaison and Recreation Specialist, and assures usage as per intention.
Assists with reporting of any repairable items to the Maintenance Department or Operations Coordinator.
Participates in the development and implementation of each resident’s active treatment plan.
Completes assessments and backup reports for each resident with the assistance of first, second and third shift staff, for all assigned IPs.
Writes active treatment programs to support the goals by the scheduled implementation date.
In-service staff on active treatment programs using demonstration, return demonstration.
Monitors staff as they do the active treatment programs for each resident; provides feedback and praise as appropriate; Assists staff in resolving issues; and provides feedback to Residential Manager on staff performance.
Assists with completion of proficiency check offs for each staff on a monthly basis as scheduled or requested.
Coordinates services with schools/worksites.
Attends all IP’s and ensures consistency of services by monitoring school sessions/workshop trainings and communicating with school /workshop personnel.
Ensures that the activity guide is up to date according to current Program Plan by the scheduled date of implementation after seeking input from the Residential Manager, Operations Coordinator, and staff.
Completes revision of programs as needed based on lack of progress after seeking input from the Residential Manager and staff.
Completes the objective data summary and plans for next month for all current STO’s.
Encourages residents to attend and participate in IP and Quarterly meetings.
Reviews IP and Residents’ Rights with resident according to their level of understanding incorporating choices and preferences.
Ensures that the residents are safe by completing safety inspections and safety in-services as requested.
Completes physical management, nutritional management and behavioral monitoring as scheduled.
Other duties as assigned.
Ensures that all documents are prepared for IP meeting by the scheduled date.
Ensures that the IP documents (IMP, Resident Profile, Schedule of Implementation, and Daily Care Summary & Procedure/Services) are turned in for typing no later than the Friday following the proposal meeting.
Proofreads draft IP documents, makes corrections and/or resolves issues; Returns the IP for placement in the Program Book before the date of implementation.
Assists with training personnel to meet residents’ needs.
Participates in performance evaluation feedbck and staff commendations as requested.
Ensures qualitative interactions between staff and residents.
Assists staff in development and training to improve their skills: Provides feedback to staff on their interactions with residents
Participates in management training; Attends in-services as requested.
Communicates with all staff about residents’ progress and needs, wishes, and desires.
Assists with completing investigations and plans of correction as requested for incident reports and acts on findings.
Assist with completing plans of correction for infection control, surveys, and returns plans to Nurse Manager or Director of Nursing as requested.
Assists with ensuring that transportation is available as needed for residents’ appointments and trips as requested.
Reviews IR notes and makes comments monthly or more often as needed.
Completes on-call responsibilities as scheduled.
Participates in grievance procedure as needed.
Participates in Case Record Review and Program Quality Review meetings as scheduled.
Works weekends and evenings as necessary.
Provides social services directly to the individuals and their families
Acts as advocate for residents and family/guardians in assuring rights are respected and residents are treated with dignity and respect.
On call 24/7 (unless otherwise designated) to notify parents/guardians of any emergency, room visits and/or hospitalizations.
Visits all homes at least twice daily.
Contacts each individual parent/guardian to gain input as to satisfaction with program/goals/care ensuring alignment of expectations with ASC values and capabilities.
Ensures individuals and family/guardians are aware of and can utilize the Grievance Process.
Maintains Grievance log ensuring grievances are addressed and filed per policy.
Ensures responses are made to all inquiries or communications from parents/guardians/surrogates within five (5) days.
Assists with pre-need planning and funeral arrangements as requested by families/guardians.
Provides leadership in prompting Self-Advocacy, Self-Determination and maintains Self-Advocacy forum minutes and annually publishes proposed agenda topics.
Guardianship Counsels individuals reaching the age of majority regarding their rights and responsibilities.
Secures the appropriate level of guardianship for any person served who does not appear to be capable of expressing informed consent.
Advocates for any individual who needs a less restrictive guardianship/health care surrogate arrangement.
Assists individuals and their guardians with governmental agencies to retain their benefits and protections.
Ensures Central Records are maintained properly and securely.
Ensures filing is up to date in conformance with Best Practices.
Monitors documentation placed in Central Records to ensure compliance; takes action on documents not in compliance.
Recruits staff advocates.
Monitors and acts on reports by advocates for concerns or suggestions.
Presents resident’s information (requests or complaints) as delivered by the individual or their advocate to the Human Rights Committee or Administration as necessary.
Oversees the coordination of meetings, notices and agenda items.
Maintains minutes of the Human Rights Committee meetings.
Assists with tour of homes as requested.
Participates in the development and implementation of the Day Program.
Assists in monitoring Day Program by observing and providing feedback to RTC’s as they perform active treatment programs for residents and assists with in-servicing, giving commendations and problem solving as needed.
Assists in ensuring that necessary materials and equipment are available for the implementation of day program activities.
Participates in ensuring that the environment is home-like, safe, and sanitary and has adequate supplies.
Ensures that all building, furnishings, and equipment are kept in good repair.
Conducts basic visual inspections to identify areas of concern with buildings, furnishings and/or equipment; Submits work orders to the Maintenance Department in a timely manner; escalates issues to supervisor if not addressed.
Ensures that all staff follows infection control guidelines.
Monitors staff/individual interactions following established metrics and quality assurance guidelines to evaluate the quality of direct care services by: Conducting weekly rounds through the houses; varying the days and shifts each week.
Regularly observing direct care activities (e.g., checking and changing, bathing, feeding, transferring individuals, etc.); Monitoring adherence to established physical, nutritional, and behavioral management guidelines.
Conducting monthly observations of the Day Program.
Reviews completed assessment tools and provides written feedback to the Residential Manager as necessary.
Reviews incident reports related to interactions between staff and individuals; Participates in Incident Report Review and Safety meetings.
Reports direct care, safety, and performance issues to Residential Manager, and PM along with recommendations to resolve issues.
Reports unusual occurrences and recommendations for resolution to the PM.
Executes professional responsibilities in an effective and efficient manner.
Participates in Agency meetings to unify departments and programs, which will ultimately benefit the Center and the people we serve.
Provides information and shares resident concerns by attending monthly management meeting and safety meetings as requested.
Maintains communication with all Circle of Support members regarding residents’ concerns and/or programs and activities.
Chairs all assigned resident meetings, IP’s and Quarterly Reviews and give feedback to PM.
