About The Position

Established in 2023, CommuniCare+OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs. Requirements

  • High School Diploma required
  • Minimum three (3) year administrative experience
  • Minimum three (3) year working in an Outpatient Primary Care Community Health Center setting or equivalent experience strongly preferred (required if no healthcare-specific education or degree)
  • strong working knowledge of Microsoft Office Suite (including Excel)
  • experience with electronic medical records systems (preferably OCHIN Epic)

Nice To Haves

  • prefer bachelor’s degree or some college coursework, especially in a health-related field.
  • Knowledge of and experience with any or all the following: Quality Improvement Methodologies (IHI Model for Improvement, Lean, Six Sigma, etc.)
  • Project Management training
  • Change Management

Responsibilities

  • Maintains a deep understanding of assigned projects (e.g. PCMH, Peer Review, PQI, Patient Experience & Satisfaction, Health Awareness Campaigns, Partnership Quality Incentive Program)
  • Collaborates effectively with the Medical Director of Clinical Quality and the QI Director to assure project goals are progressing as planned and necessary adjustments communicated and made as needed in a timely manner
  • Orients and informs staff as needed on key QI projects, assuring understanding of aims and how to improve, to encourage a culture of quality improvement
  • Advise improvement efforts by engaging in discussions and activities with appropriate staff around key improvement needs areas, assuring timelines are effectively communicated, so project goals are met on time
  • Assure documentation of assigned projects are complete and accurate, according to the requirements of the project sponsor (internal or external)
  • Develop, track, and communicate outcome and process measures to participating project team in meaningful ways to inform, encourage, and enable improvements.
  • Communicate data in meaningful and understanding ways both verbally and through Excel, PowerPoint presentations. Must be able to tailor information to different audiences and groups.
  • Gather, summarize, and present information about project requirements, progress, and outcomes at appropriate meetings (Site Staff meetings, QI Meetings, etc.) as assigned.
  • Performs other duties as assigned.
