About The Position
Impact Health is looking for a Psych Nurse Practitioner (Full Time) to join our Team in Monroe, CT. Impact Health PC is a leading advanced practitioner group in skilled nursing facilities across the Northeast, dedicated to prioritizing patient care. We specialize in Medical, Psychiatric Services, Wound Care, and Palliative Care. This role offers a collaborative environment that values expertise and independence, with the freedom of a flexible Monday through Friday schedule, no nights, weekends, or holidays. We are seeking an experienced NP or PA for a flexible, well-supported role.
Requirements
- Licensed Family, Geriatric, or Psychiatric Nurse Practitioner in the state of practice.
- Minimum of 2 years of clinical experience providing mental health care to adult populations.
- Proficient with EHRs and clinical technology
- Strong communicator with a collaborative mindset
Nice To Haves
- Experience in skilled nursing facilities, hospitals, residential care, geriatric services, and outpatient psychiatric settings preferred.
Responsibilities
- Deliver person-centered care using evidence-based practices
- Manage caseload and fulfill psychiatric consultation needs of skilled nursing facility
- Provide assessment, diagnostic, and treatment services
- Provide psychoeducation and medication education to patients and/or families
- Collaborate with interdisciplinary facility teams
- Complete timely documentation (within 24 hours)
- Ensure compliance with state regulations in SNF settings
