About The Position

Impact Health is looking for a Psych Nurse Practitioner to join our Team in Hartford, CT. Impact Health PC is a leading advanced practitioner group in skilled nursing facilities across the Northeast, dedicated to prioritizing patient care. We specialize in Medical, Psychiatric Services, Wound Care and Palliative Care. This role is specifically for weekend coverage in our friendly skilled nursing facilities. We are seeking an experienced NP for a flexible, well-supported role.

Requirements

• Licensed Psychiatric Nurse Practitioner in the state of practice.
• Minimum of 2 years of clinical experience providing mental health care to adult populations.
• Proficient with EHRs and clinical technology
• Strong communicator with a collaborative mindset

Nice To Haves

• Experience in skilled nursing facilities, hospitals, residential care, geriatric services, and outpatient psychiatric settings preferred.

Responsibilities

• Deliver person-centered care using evidence-based practices
• Manage caseload and fulfill psychiatric consultation needs of skilled nursing facility
• Provide assessment, diagnostic, and treatment services
• Provide psychoeducation and medication education to patients and/or families
• Collaborate with interdisciplinary facility teams
• Complete timely documentation (within 24 hours)
• Ensure compliance with state regulations in SNF settings
