$150,000 - $250,000Hybrid

About The Position

Skilled Physicians Group is a dedicated clinical team committed to bringing high-quality, compassionate care to patients in skilled nursing and long-term care facilities. We believe every patient — regardless of where they call home — deserves access to exceptional psychiatric care. We are expanding our team and looking for a mission-driven Psychiatric Nurse Practitioner to join us. This is more than a job — it is a calling. As a Psychiatric NP with Skilled Physicians Group you will serve one of the most underserved patient populations in healthcare, bringing expert psychiatric care directly to residents in skilled nursing and long-term care facilities. You will operate as a mobile NP, making rounds across facilities and serving as a vital member of an interdisciplinary care team. If you are passionate about geriatric psychiatry, behavioral health, and making a meaningful difference in the lives of vulnerable patients — this role was built for you.

Requirements

  • Active and unrestricted Nurse Practitioner license in good standing
  • Psychiatric Mental Health Nurse Practitioner (PMHNP) certification required
  • Strong clinical skills in psychiatric evaluation and psychotropic medication management
  • Comfortable working in a mobile, facility-based care model
  • Compassionate, patient-centered approach to care
  • Excellent communication and interdisciplinary collaboration skills
  • Self-motivated with the ability to manage your schedule and prioritize patient needs independently

Nice To Haves

  • Experience in geriatric psychiatry, long-term care, or skilled nursing settings preferred — but a genuine passion for this patient population is equally valued

Responsibilities

  • Conduct comprehensive psychiatric assessments for long-term care and skilled nursing facility residents
  • Provide ongoing evaluation and management of psychotropic medications with a focus on appropriateness, safety, and reduction where clinically indicated
  • Deliver evidence-based psychotherapy services tailored to the unique needs of the long-term care population
  • Participate in fully integrated interdisciplinary care plan meetings focused on reducing psychotropic medications, addressing behavioral challenges, and tracking resident progress toward treatment goals
  • Partner closely with physicians, nursing staff, social workers, and facility leadership to ensure a whole-person approach to psychiatric care
  • Travel between skilled nursing and long-term care facilities in your region to deliver consistent, high-quality psychiatric services

Benefits

  • Malpractice coverage fully provided
