About The Position
Skilled Physicians Group is seeking a mission-driven Psychiatric Nurse Practitioner to join their dedicated clinical team. This role focuses on bringing high-quality, compassionate psychiatric care to patients in skilled nursing and long-term care facilities, serving an underserved patient population. The Psychiatric NP will operate as a mobile provider, making rounds across facilities and collaborating with an interdisciplinary care team. This position is ideal for individuals passionate about geriatric psychiatry, behavioral health, and making a meaningful impact on vulnerable patients.
Requirements
- Active and unrestricted Nurse Practitioner license in good standing.
- Psychiatric Mental Health Nurse Practitioner (PMHNHP) certification required.
- Comfortable working in a mobile, facility-based care model.
- Compassionate, patient-centered approach to care.
- Excellent communication and interdisciplinary collaboration skills.
- Self-motivated with the ability to manage your schedule and prioritize patient needs independently.
Nice To Haves
- Experience in geriatric psychiatry, long-term care, or skilled nursing settings preferred.
- A genuine passion for this patient population is equally valued.
Responsibilities
- Conduct comprehensive psychiatric assessments for long-term care and skilled nursing facility residents.
- Provide ongoing evaluation and management of psychotropic medications with a focus on appropriateness, safety, and reduction where clinically indicated.
- Deliver evidence-based psychotherapy services tailored to the unique needs of the long-term care population.
- Participate in integrated interdisciplinary care plan meetings focused on reducing psychotropic medications, addressing behavioral challenges, and tracking resident progress.
- Partner closely with physicians, nursing staff, social workers, and facility leadership to ensure a whole-person approach to psychiatric care.
- Travel between skilled nursing and long-term care facilities in your region to deliver consistent, high-quality psychiatric services.
