About The Position

The PSR/PSI Caseworker provides psychosocial rehabilitation for adults with a serious mental illness and or co-occurring mental illness and substance abuse disorder while they attend a day support program. Utilizes a variety of approaches and interventions to promote recovery and assists in successful community integration experiences. Encourages consumers with participation in meaningful psychosocial activities, work and/or training. Promotes mental health recovery by coordinating recovery-based psycho-educational classes designed to empower consumers in their journey of recovery and transformation. Promotes access to treatment by creating a welcoming and accessible environment.

Requirements

  • High school diploma or high school equivalent.
  • Two (2) years of any combination of experience including working with communities on health or social service related matters, social work/case management experience, behavioral health and/or health care.
  • Must have the ability to assess and utilize community resources.
  • Required to attend in-service training sessions designed to provide training in accordance with GCLC, DOH and Medicaid Standards.
  • Must be able to pass Criminal Records Check and receive CYFD Clearance.
  • Maintain a valid and unrestricted NM Driver’s License.
  • Must be able to use personal vehicle over course of employment.
  • Maintain valid and sufficient personal automobile insurance.
  • Must pass and maintain a Driving Record clearance in accordance with GCLC policies and procedures.

Nice To Haves

  • Bachelor's Degree in Social Work, Education, Counseling, Psychology, Sociology, Criminal Justice, Family Studies, or a related degree.
  • Experience working with adults with Severely Disabling Mental Illness (SDMI) preferred.
  • Previous work providing in-home or community-based mental health or case management services.
  • Experience with medically integrated mental health services.
  • Demonstrated interest in rural and community health.
  • Fluency in English and Spanish, both verbal and written.

Responsibilities

  • The implementation and service delivery will include but not be limited to, personal budgeting and financial management, basic living skills, coping skills, personal hygiene, comparative shopping, stress management, etc.
  • Responsible for teaching independent living skills as developed by the Psychosocial Rehabilitation Team in compliance with contractual, Medicaid, Medicare, Department of Mental Health and GCLC requirements.
  • Assist client with learning to compensate for the effects of the symptoms of the illness through the development of new skills and a supportive environment and to counteract the effects of secondary symptoms by restoring a sense of confidence.
  • Responsible for ensuring the safety of all clients and leading meaningful activities within the program milieu.
  • These activities may include but are not limited to therapeutic socialization, classes, community meeting, and other such events that together constitute the schedule of the integrated program model.
  • Proper documentation of services including the beginning and ending time, the duration of billable services, the client’s daily goal and description of the participation that day.
  • Responsible for providing transportation services as needed for clients.
  • Requisite, supervise and facilitate recreational and social activities.
  • Must be able to demonstrate the ability to assist clients with disabling mental illness to achieve their maximum level of community integration and self-reliance.
  • Maintains confidentiality of all information regarding clients, staff, physicians, and GCLC obtained as a result of employment.
  • Other duties as assigned.
