About The Position
In this innovative project Care Managers will bridge the gap between Adult Protective Services (APS) and community-based aging services. Care Managers work with APS to identify and serve older adults who would benefit from programs and opportunities that are available within the community. Care Managers offer a holistic assessment of the client’s needs by identifying health, financial, emotional, functional and environmental concerns and will offer care options, solutions and guidance and connecting with appropriate programs and systems. This job posting is for a specialized Care Manager (CM) who will participate in a grant program designed to integrate aging services with Adult Protective Services (APS). This CM will act as a consultant to APS staff at Monroe County Department of Human Services concerning aging services for older APS clients, including services at Lifespan, will conduct joint assessments with APS staff in some cases, and will help to develop service plans, engage APS clients in services and help clients transition to care management services in Lifespan’s Community Care Connections (CCC) program. The CM will as act as the lead CM for the CM group assigned to this project, will facilitate new referrals to the program and will assist in coordinating the overall care management work of the project.
Requirements
- Bachelor or Master of Social Work or equivalent degree.
- Two years in social work, with at least one-year experience in geriatrics.
- Must be able to meet with clients in their homes.
- A portion of the work is performed at a desk or at a personal computer station.
- Verbal, written and telephone communication skills are required.
- Strong written and computer skills needed.
- Interface with all levels of personnel.
- Ability to work in an Adult Protective Service environment and with older adult clients at risk of abuse, neglect, exploitation and self-neglect.
- Ability to maintain composure under strict deadlines and difficult situations.
- Ability to work in multi-cultural situations.

Responsibilities
- Provide care management to clients.
- Accepts referrals as assigned.
- Completes comprehensive in-home assessments to determine needs.
- Creates and implements client-centered care plans in conjunction with APS, as appropriate, that meet client needs and preferences.
- Provides information and referral, advocacy, benefit and entitlement counseling.
- Records case activity in the Lifespan database designated for the project.
- Arranges mental health interventions for older adults and their caregivers, when necessary.
- Conducts mental health screenings when appropriate.
- Provides supportive counseling to older adults.
- Works in tandem with APS to identify and provide linkage to appropriate ongoing mental health community services at intake.
- Participates as a member of the inter-agency team.
- Acts in the capacity of lead Care Manager for project activities and assists in coordination of the activities of other care management staff assigned to the project.
- Participates in all Community Care Connections team meetings and trainings.
- Participates in all Lifespan staff meetings and trainings.
- Participates in quality assurance activities.
- Provides input into the development of this pilot program.
- Accepts and seeks guidance and clinical supervision.
- Maintains up to date professional knowledge of the geriatric field, community services entitlement benefits, clinical skills and competencies through reading, workshops and conferences.
- Participates in training required for grant project activities including Motivational Interviewing, Trauma Informed Care and Racial Equity training.
- Is aware of and actively supports the Agency’s initiative of being collaborative and community focused, innovative and culturally competent.
- Other duties as assigned by supervisor.
