Program Support Assistant - Case Management
About The Position
The case management associate participates on the Case Management Team (CMT), with the guidance of the nurse case manager and social worker, assists in facilitating appropriate patient placement; utilizes hospital and community resources for continuity of patient care; serves as Utilization Management liaison; collaborates with Medical staff, CMT members, Business Office, Medical Records and other hospital staff; performs other duties as assigned. Requirements
- 1 year job related experience in a healthcare setting.
Nice To Haves
- Associate's Degree
- Bachelor's Degree
- Nursing Assistant Certification (CNA), Health Unit Coordinator (HUC), Medical Terminology
- Prior experience in discharge planning, billing, utilization review, financial counseling or registration.
- Proficiency in medical terminology.
Responsibilities
