About The Position

As Case Management Support, you will work with other dedicated professionals who share your passion for helping individuals in need. Our Communities Connected for Kids Program provides services for children and families in the child welfare system, to ensure a community without abuse, neglect and abandonment. Based out of Fort Pierce, FL, this position is responsible for supporting Dependency Case Managers with assigned tasks, transporting clients to and from court appointments, and medical appointments, coordinating and supervising family visitations, supervising children, and documentation of those appointments.

Requirements

  • High School diploma or GED equivalent required
  • 1+ years related experience required (preferably working with children)
  • Valid Driver's License in good standing required

Nice To Haves

  • 2+ years of experience working with children or families preferred
  • Previous experience in a social service organization preferred
  • Prior experience installing children's car seats, types of seats, requirements, and expiration dates
  • Class B CDL License with passenger endorsement a plus

Responsibilities

  • Provide transportation for dependent children and parents if needed to medical appointments, counseling, court, placements etc.
  • Provide long distance transportation (may result in overnight stay for employee)
  • Supervise children.
  • Coordinate visitations between case managers, foster parents, parents, children and caregivers.
  • Transport and supervise visitation between children, parents, caregivers, etc. (Majority of job)
  • Document in electronic case file all contacts made as well as observations, interactions, behaviors of children, parents and caregivers.
  • Assist Dependency Case Managers in providing case management services (information, referral and linkages).
  • Assist Dependency Case Managers in obtaining records and documents needed for the delivery of services (e.g., education records, medical/dental records, vital statistics, etc.)
  • Testify in court hearings regarding observations/documentations.
  • Effective time management.
  • Attend staff meetings.
  • Be available to work after hours and/or on weekends as needed.
  • Effective verbal and written communication.
  • Participate in the accreditation and re-accreditation processes.
  • Perform other duties as assigned.
