About The Position

Under the direction of the Asst. Director, the Program Navigator assists in the coordination and implementation of long-term services and support in the community. At CCC, all duties are performed in a manner that fosters the achievement of the organization’s mission.

Requirements

• High School Diploma required.
• Project management and organizational skills.
• Strong verbal and written communication skills.
• Ability to maintain confidentiality of client and company information.
• Ability to understand and articulate multiple programs.
• Reliable transportation, valid driver’s license and current automobile insurance.

Nice To Haves

• Associate Degree in Health or Human Service preferred.
• For High School graduates, at least two years’ experience in a human service setting preferred.
• For attained Associate Degrees, at least one year experience in a human service setting, or equivalent experience preferred.
• Knowledge of community resources preferred.
• Working knowledge of Connexus.

Responsibilities

• Provides Referral Screening for all new potential clients of Public Programs supported by CCC.
• Provides telephonic communication with client and/or the referral source to provide education and program navigation.
• Schedules initial assessments.
• Follows, supports and assists pending client's post-assessment to assist with timely activation onto appropriate program/s. This will include home visits as needed.
• Participates in and works effectively as part of an interdisciplinary team.
• Assist in mentoring new staff as needed.
