About The Position

Responsible for the day-to-day operations of Ryan Health Network Sites HIV Services programs. Provides direct supervision to the Case Manager (including the Case Manager at Ryan Chelsea-Clinton), Care Technician, Data Entry staff, and Peer Educator. Ensures that patients with targeted chronic conditions receive the highest quality care and support services. The target population includes, but is not limited to, patients over age 50 who are experiencing health disparities and need care coordination to achieve improved and sustained health outcomes, as well as access to necessary medical, behavioral health, and social services. Ensures that the Over 50 program remains compliant with all program expectations and meets quality indicators, including productivity targets. Also ensures that patients who have fallen out of care, or who are at risk of falling out of care, are connected to and maintain continuous access to case management, medical, psychiatric, and supportive services in collaboration with the HIV Services team. This work is carried out in support of the mission and goals of Ryan Health. This is a grant-funded role ending February 28, 2029.

Requirements

  • Three years of experience working with chronic care populations in a medical setting, including behavioral health and HIV populations.
  • Three years of clinical and administrative experience in social work or public health, preferably within a medical setting.
  • Working knowledge of Microsoft Office Suite, as well as database management.
  • Master’s degree in Social Work, Master’s degree in Public Health, or Master’s degree in Social Services or a related field.

Responsibilities

  • Supervise and coordinate case management, case-finding, monitoring, and follow-up activities conducted by Case Managers, Care Technicians, Data Entry Clerks, and Peer Educators. Responsibilities include recruitment, hiring, onboarding, training, scheduling, attendance monitoring, performance evaluation, disciplinary action, and providing day-to-day support to ensure staff meet established standards for service delivery, professionalism, and productivity.
  • In collaboration with the Senior Director of Support Services and Physician Assistant Supervisor, facilitate weekly case conferences and consult with interdisciplinary team members and external service providers to develop and implement unified service plans that ensure coordinated and comprehensive medical, psychiatric, and supportive care for Harlem patients.
  • Analyze program data, prepare program reports, conduct chart reviews, monitor program outcomes, and provide recommendations for quality improvement and enhanced service delivery.
  • Assist with program planning, development, and implementation to address the evolving needs of patients served.
  • Develop and maintain partnerships with external agencies, including mental health providers, substance use treatment programs, social service agencies, and community-based organizations, to support patients’ psychosocial needs.
  • Collaborate with fiscal leadership to support budget management, monitor program projections, and ensure alignment with program goals and funding requirements.
  • Coordinate staff and program operations, including case assignments, admissions, discharges, and ongoing case monitoring.
  • Participate in meetings with primary care and interdisciplinary staff to monitor program implementation and patient care coordination, including monthly provider meetings and daily huddles, as appropriate.
  • Identify staff training and professional development needs, and provide ongoing coaching, supervision, and support to strengthen staff performance and service quality.
  • Participate in professional development opportunities and remain current on evidence-based practices relevant to case management and supportive services.
  • Serve as a super-user for all electronic systems utilized by program staff for patient documentation and data management.
  • Coordinate staff trainings and develop internal training materials and procedures as needed.
  • Participate in patient eligibility assessments, review staff assessments, and provide clinical recommendations within scope of practice.
  • Assist with proposal development, grant writing, abstracts, presentations, and new program initiatives.
  • Support the development and implementation of network-wide in-service training programs to enhance staff competencies and service delivery standards.
  • Assist staff and administrative monitors in functioning effectively as members of the interdisciplinary care team.
  • Attend and actively participate in required meetings, case conferences, trainings, and supervision sessions.
  • Maintain accurate and timely documentation related to supervisory activities, meeting minutes, quality improvement initiatives, and clinical operations.
