NewYork-Presbyterian (NYP) is seeking a Program Manager to oversee its care coordination programming and Health Home program activities. This role is within the Comprehensive Health Program (CHP), a center of excellence for HIV treatment and prevention, sexually transmitted infections, and general infectious diseases. CHP focuses on addressing social determinants of health and mitigating risks for vulnerable populations through comprehensive care coordination, multidisciplinary team-based care, and integrated behavioral health services. The Health Home program aims to address complex medical conditions and medical, behavioral, and social needs comprehensively, with care teams developing patient-centered care plans. The Program Manager will lead these programs alongside other managers, overseeing daily operations, monitoring program requirements (billing, documentation, data reports), and guiding the strategic vision for sustainability. Collaboration with team members, including supervising care coordinators, is a key aspect of the role. The ideal candidate will possess strong organizational skills, the ability to interpret data reports for high fidelity, and familiarity with grant-funded program implementation, evaluation, and management. Effective supervision of care coordinators and excellent communication skills (verbal and written) are crucial. Experience in case management and/or behavioral health is beneficial for guiding care coordinators. Program Managers participate in the Care Coordination Steering committee and collaborate with various NYP departments and community organizations to ensure program success and visibility. They will also track and report program outcomes to funders, senior leadership, and stakeholders.
