About The Position

The Health Educator is responsible for delivering individual and group health education services, supporting community wellness initiatives, and promoting health equity through culturally responsive education and outreach. The position develops health education materials, conducts patient and community engagement activities, collaborates with clinical and outreach teams, and supports program evaluation efforts to improve health outcomes and access to care.

Requirements

  • Associate degree in community health, mental health, nutrition, or related field, or equivalent work experience.
  • Minimum 2 years of experience in health education, wellness programming, or community outreach.
  • Fluency in English and Spanish required.
  • Strong interpersonal and written communication skills.
  • Ability to communicate clearly using plain language.

Nice To Haves

  • Project management experience preferred.
  • Fluency in Arabic, Dari, Pashto, Rohingya, and Burmese is highly desired.

Responsibilities

  • Conduct patient outreach and engagement activities to increase participation in health education programs, wellness initiatives, preventive services, and other organizational programs.
  • Develop, maintain, and distribute culturally responsive, health-literate educational materials that support patient understanding, self-management, and informed healthcare decision-making.
  • Facilitate individual and group health education sessions for patients and community members.
  • Collaborate with outreach, clinical, and population health teams to design and lead wellness initiatives.
  • Promote health equity through culturally responsive and size-inclusive education.
  • Develop and distribute plain-language educational materials on various wellness topics.
  • Coordinate with local organizations and partners to expand access to health and wellness programs.
  • Track participation, outcomes, and generate reports to inform programming and improve effectiveness.
  • Support patients in overcoming barriers to wellness such as food access, transportation, and health literacy.
  • Promote integrative care and refer individuals to primary care, mental health, and community resources.
  • Participate in program evaluation and continuous improvement efforts.
  • Other responsibilities as assigned.
