About The Position

Sentara Community Care Clinic is hiring a Program Manager for a rewarding opportunity to give back to the community by supporting meaningful health initiatives and improving access to care! Plan, develop, implement, and manage assigned outreach programs for Sentara. Collaborate with community stakeholders to increase access to services, raise awareness of outreach initiatives, and provide comprehensive support to individuals and families. Coordinate outreach activities, special projects, and community partnerships that promote health, wellness, and nutritional support needs. Develop and maintain relationships with local agencies, donors, suppliers, and community organizations to ensure access to resources for identified populations. Collect and report program data and metrics. Serve as an advocate and educator while connecting community members to available resources and support services. Oversee day-to-day operations of assigned outreach programs. Assist with identifying and addressing health and nutritional needs of patients and the community.

Requirements

  • Bachelor’s degree in Social Work, Public Health, Nonprofit Management, Human Services, or related field required OR 3 years of relevant experience in community outreach, social services, food pantry operations, or related field in lieu of degree
  • Experience in community outreach programs, food pantry management, social services, or related field required
  • Experience working with community-based organizations and support agencies required

  • Strong communication, education, and advocacy skills
  • Ability to engage and build relationships with diverse populations and community stakeholders
  • Strong organizational and program coordination skills

Responsibilities

  • Conduct public awareness and outreach activities to promote programs and services
  • Support development and implementation of outreach programs and community events
  • Assist in identifying community health and nutritional needs and connecting individuals to resources
  • Collect, analyze, and report program data, metrics, and outcomes
  • Maintain relationships with community partners, agencies, donors, and stakeholders
  • Ensure effective coordination of outreach initiatives and special projects
  • Oversee daily operations of assigned outreach programs
