Prava Therapy•Sierra Madre, CA
•$48 - $54•Onsite

About The Position

Prava Therapy is seeking a Program Director, who is a licensed Physical Therapist or Occupational Therapist, to lead their therapy program at The Kensington Sierra Madre in Sierra Madre, California. This role is for a clinician who desires to do more than just treat patients; they want to lead, build, mentor, connect, and influence, while still maintaining their clinical practice. The Kensington Sierra Madre is an enhanced assisted living and memory care community that provides a high level of clinical support in a home-like setting, including 24/7 nursing, on-site rehabilitation, wellness resources, and specialized memory care. This environment allows therapy to be an integral part of helping residents live well. As Program Director, you will be the clinical and community leader for the therapy program, maintaining your own caseload while leading the team and ensuring exceptional resident care. You will build relationships across the community, understand residents' needs, and foster a collaborative environment. The role involves coaching therapists, participating in interdisciplinary discussions, and ensuring therapy goals align with residents' daily lives and aspirations. You will support residents with various needs, partner with caregivers, monitor clinical quality, and identify opportunities for program improvement. The goal is to make therapy visible throughout the community and represent Prava as a clinical partner.

Requirements

  • Understanding of the person in front of you, their values, what they are trying to get back to, what matters to their family, and what is getting in their way.

  • Belief that residents should be appropriately challenged—not underestimated.
  • Belief that outcomes matter more than minutes.
  • Belief that caregiver education is part of treatment.
  • Belief that dignity and autonomy are clinical priorities.
  • Belief that the best therapy programs are built through relationships.

  • Desire to be visible and embedded in a senior living community.
  • Desire for clinical expertise to influence how an entire program operates.
  • Energized by building something better—not maintaining the status quo.

Responsibilities

  • Ensure therapy goals are connected to what residents actually want and need to do in their daily lives.
  • Help clinicians think beyond impairments and minutes toward function, participation, independence, and quality of life.
  • Support residents with a wide range of needs, including mobility limitations, falls, neurologic conditions, orthopedic recovery, cognitive impairment, dementia, and changes in functional status.
  • Partner with caregivers and community staff to reinforce strategies that help residents function safely and confidently throughout the day.
  • Monitor clinical quality, documentation, outcomes, scheduling, caseload trends, and other key program indicators.
  • Identify opportunities to strengthen the therapy program and work with Prava leadership to turn those opportunities into action.
