Prava is seeking an entrepreneurial Physical Therapist Practice Owner to build and lead a thriving therapy practice within a premier senior living partner community in Northern California. This role is embedded within the resident experience, focusing on building relationships, identifying therapy opportunities, developing referral pathways, and leading clinicians to support residents' desire for mobility, participation, connection, and independent living. Unlike traditional outpatient clinics, this position involves a deep integration into the community's culture and resident life.

Requirements

Licensed Physical Therapist in California
Experienced clinician who wants more ownership over their practice
Entrepreneurial and energized by building something
Comfortable working independently while collaborating closely with a larger team
Strong communicator who naturally builds relationships
Passionate about senior living, aging well, and functional independence
Interested in developing other clinicians as the practice grows
Someone who sees the opportunity in senior living—not simply the limitations of it

Responsibilities

Establish and grow the physical therapy program within the community.
Develop strong relationships with residents, families, and community leadership.
Build referral pathways and create awareness of the clinical services available.
Identify opportunities to expand services as resident needs evolve.
Evaluate and treat residents across the continuum of senior living.
Establish a high standard of clinical reasoning, documentation, and resident-centered care.
Mentor and develop additional clinicians as the practice grows.
Create a culture where quality matters as much as volume.
Partner with the community's wellness and fitness teams.
Look beyond the traditional therapy episode and think about mobility, participation, and long-term function.
Help residents remain engaged in the activities, relationships, and routines that matter to them.
Bring clinical expertise into the broader senior living environment.
Understand your practice's clinical and financial performance.
Monitor volume, utilization, referrals, and growth.
