About The Position

Prava partners with exceptional clinicians to build and operate concierge-style therapy practices inside senior living communities. We provide the technology, billing, credentialing, operations, marketing, referrals, and business infrastructure. We’re looking for a Physical Therapist Practice Owner to lead a thriving therapy program within a memory care community — someone who understands that exceptional therapy in memory care looks different, requires different clinical reasoning, and demands a deep understanding of the resident, the environment, and the people supporting them. This is not a traditional PT job. You won't be walking into a schedule and simply treating the next resident. You will have the opportunity to build something. As the Practice Owner, you will become the clinical leader and face of the therapy practice within the community. You'll develop relationships with residents, families, caregivers, nurses, physicians, leadership, and community partners while building a program that becomes an integral part of the community.

Requirements

Licensed Physical Therapist
Strong clinical reasoning and communication skills
Genuine interest in senior living and memory care
Entrepreneurial mindset
Comfortable being visible and accountable within a community
Ability to build relationships and earn trust
Strong documentation and clinical standards
Desire to lead rather than simply treat
Willingness to make a full-time commitment to the practice and community

Responsibilities

Lead the physical therapy program within a dedicated memory care community
Maintain a highly visible, consistent presence in the community
Evaluate and treat residents using strong clinical reasoning and individualized care plans
Develop meaningful relationships with residents, families, caregivers, and community leadership
Identify residents who may benefit from skilled therapy and develop appropriate referral pathways
Build a therapy culture focused on function, mobility, safety, participation, and quality of life
Collaborate with nursing, wellness, activities, physicians, families, and other care partners
Mentor and lead additional clinicians as the practice grows
Monitor clinical outcomes, utilization, documentation, and practice performance
Participate in Prava's operating rhythm, including weekly coaching and reporting
Help identify opportunities to expand services and better serve the community
Become the clinical expert and trusted therapy resource for the community
