Prava Therapy•Elk Grove, CA
•$50 - $56•Onsite

About The Position

This role is for a Director of Rehabilitation to lead and grow a comprehensive rehabilitation program within a premier Tenfold Senior Living community in Northern California. The position involves leading the clinical environment, building a strong team, establishing clinical standards, partnering with community leadership, and creating a rehabilitation program that is recognized as an essential part of the community. The model emphasizes wellness, mobility, autonomy, and helping residents remain engaged in their community, focusing on outcomes like walking to dinner, getting to fitness class, participating in activities, going out with friends, moving safely around their apartment, and maintaining independence. Prava provides the infrastructure, including EHR and practice management technology, AI-enabled documentation tools, revenue cycle management, credentialing and payer support, recruiting infrastructure, marketing and referral support, clinical leadership and coaching, reporting and business intelligence, and operational support.

Requirements

  • A licensed rehabilitation clinician in California
  • An experienced PT, OT or SLP with demonstrated leadership ability
  • Comfortable leading clinicians while maintaining your own clinical practice
  • A strong clinical thinker who cares about quality—not just completed visits
  • A relationship builder who works naturally with senior living leadership
  • Interested in mentoring and developing other clinicians
  • Organized, accountable and comfortable using data to manage performance
  • Excited by the opportunity to build something rather than simply inherit a department

Nice To Haves

  • Senior living experience is valuable, but leadership ability, clinical judgment and the desire to build a great program matter just as much.

  • Help position rehabilitation as part of the resident's broader lifestyle and independence—not simply a service delivered after a decline.
  • Monitor census, referrals, evaluations, visits and clinical utilization.
  • Identify opportunities to expand appropriate services.
  • Help develop new service lines as community needs evolve.
  • Use Prava's reporting and technology to understand what is happening inside the practice and where intervention is needed.
