Lead Licensed Physical Therapist
About The Position
This position is for a Physical Therapist and clinical leader who sees senior living differently. The community is built around movement, engagement, independence, and exceptional experiences, with residents participating in fitness, social programs, outings, dining, education, and daily life. The community also supports Assisted Living, Memory Care, restorative care, and specialized Parkinson's programming. This creates an incredible environment for a PT who understands that mobility is about much more than gait and strength. As Director of Rehabilitation, you'll lead the physical therapy program while remaining clinically engaged. You'll develop your team, elevate clinical reasoning, build relationships across the community, and partner with leadership to make rehabilitation an integral part of the resident experience. Prava provides the technology, clinical infrastructure, documentation tools, RCM, credentialing, operational support, and resources that allow great clinicians to focus on leading great care. Responsibilities
- Lead and develop an exceptional therapy team
- Maintain an active clinical caseload
- Drive clinical quality, outcomes, and evidence-informed practice
- Build strong relationships with residents, families, caregivers, nursing, and community leadership
- Develop programming around mobility, balance, fall prevention, strength, function, and independence
- Support residents across Assisted Living, Memory Care, and wellness programming
- Identify opportunities for early intervention and proactive clinical support
- Build strong referral and interdisciplinary relationships
- Use data and clinical insight to continually improve the program
