The Program Coordinator is responsible for providing direct supervision, coaching, feedback, and evaluation to Residential Managers. This role also involves assisting the Director of Residential Services with payroll preparation and ensuring the proper maintenance of apartments and homes to meet regulatory requirements and support the lifestyle of the individuals served. The Program Coordinator plays a key role in the Person-Centered Individual Service Plan (ISP) process, including its development, planning, and implementation, and monitors resident progress towards objectives, adjusting strategies as needed. This position also involves conducting assessments, ensuring the completion of various ISP components, leading Person Centered Planning meetings, and collaborating with interdisciplinary teams. The role includes training staff, overseeing new staff orientation, acting as a liaison between families, guardians, and external agencies, and managing all pertinent information related to individuals served. Additionally, the Program Coordinator coordinates service delivery, manages case management, completes mental health assessments and reports, attends psychiatric appointments, supervises waiver authorizations, coordinates with day programs and work locations, oversees health care functions, assists with managing individual funds, manages data collection, and prepares for monitoring, surveys, and audits. The position also requires being on call after business hours for emergencies and performing other assigned duties.
