About The Position

The Program Coordinator of Kinship Care is responsible for coordinating all aspects of Kinship Care Programs, including home visits, working with case managers, maintaining documentation, managing files and other administrative processes.

Requirements

  • The equivalent of 2 to 3 years of customer service and administrative experience
  • Proficiency in various software applications including Word, Excel, Access to promote efficient communication and accurate record keeping
  • $100,000/300,000 Automobile Liability Insurance
  • Valid NH driver’s license and access to reliable vehicle during working hours to transport individuals
  • Demonstrate a commitment to The Moore Center’s values
  • Understand and comply with all state, agency and department policies and procedures
  • Possess strong time management and organization skills, including the ability to prioritize multiple tasks and ensure timely follow through
  • Ability to work independently and as part of an inter-disciplinary team

  • Interact effectively with people of varied educational, socioeconomic and ethnic backgrounds, skill levels and value systems
  • Demonstrate the ability to encourage people

  • Ability to provide well-written, accurate and timely reports that ensure compliance with all regulations and established practices
  • Report suspected incidents of client abuse, neglect, and/or exploitation
  • Ability to speak, read and write English
  • Maintain criminal, motor vehicle and Bureau of Elderly and Adult Services record checks that meet agency standards
  • Travel and transport clients when applicable
  • Having access to a reliable vehicle
  • Maintaining personal auto insurance of $100,000/300,000

Responsibilities

  • Conduct quarterly home visits of all Kinship Care programs
  • Meet with family provider, review policies, walk through home (or conduct on phone), make recommendations if needed
  • Collect and process documentation for Kinship Care
  • Work closely with Case Managers; provide verbal and written communication on regular basis
  • Update care plans, update contracts, perform safety checks, track Progress Notes/ Fire Drills forms
  • Track Client/Provider changes in software
  • Forward updates of client status changes to Manager
  • Ensure quality of service in Kinship Care homes
  • Conduct home visits quarterly
  • Maintain up-to-date, accurate and complete client records in accordance with program policy
  • Assist Manager with program compliance
  • Teamwork, regular attendance in the office, and face-to-face interaction (as needed) are essential job functions
  • Other duties and responsibilities as assigned by supervisor
