About The Position

We are seeking a full-time Kinship Care Coordinator role to join our Care Management Team in Columbus, Ohio. The Kinship Care Coordinator works collaboratively with Social Workers in family intervention to provide an array of supportive and educational services. Requirements

  • Bachelor’s Degree in Social Worker or a related Human Services field
  • Two years’ professional experience working in kinship care programs
  • Knowledge of Kinship Care Best Practice Models and Utilization of Navigator Equipment
  • Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
  • Excellent customer service and communication skills
  • Works well independently and as a team member
  • Multi-task efficiently and be flexible in all situations

Responsibilities

  • Complete initial walkthroughs, home study assessments, background checks, and all other required paperwork necessary to facilitate a Kinship Care Program placement.
  • Educating the kinship placements on reunification efforts and setting appropriate boundaries with the parents of the of the child(ren) placed in their care
  • Develop safety plans with kinship placements as necessary
  • Document all contact with clients and collaterals, and complete case activities
  • Engage families, assess the needs related to skill training, provide education, demonstrate resource enhancement, and develop a supportive relationship with families
  • Teach and provide support through demonstration, modeling, written materials, videos, and parent groups
  • Establish connections for families with necessary resources, and use listening, interviewing, and problem solving techniques
  • Coordinate with the assigned Social Worker to develop a service plan and assess the achievement of service goals and teach home management techniques and monitors client’s progress toward treatment goals
  • Complete unannounced home visits to monitor in home safety plans and provide safety services for all children and families
  • Coordinate, organize and order all car seats used by Family Services and ensure the safety of each seat
  • Work collaboratively with other NYAP programs and staff throughout the organization
  • Perform all duties in accordance with personnel policies and the program’s fiscal guidelines
