About The Position

Bridges Healthcare, Inc. has an immediate opening for a long-term temporary Kinship Navigator. $27.00 per-hour/25-35 hours per-week, Monday through Friday. This is a non-benefited position, and the duration could be up to 1 year. PURPOSE and POSITION OVERVIEW Maintain children in kinship and fictive kin care by providing collateral and case management services.

Requirements

• Minimum 2 years lived experience as either kinship caregivers or have lived in a household that provided kinship care required
• Ability to work flexible schedule.
• Knowledge of community integration strategies and techniques.
• Good communication and documentation skills.
• Computer literacy.
• Ability to prioritize and organize schedules/demands.
• CPR/First Aid certified or willing to become certified.
• Valid driver's license and access to reliable transportation required.

Nice To Haves

• Bachelor's degree preferred.

Responsibilities

• Conducts initial and comprehensive assessment of children and their kinship caregivers to identify service needs.
• Provide in home-based support services to children and families.
• Demonstrate a comprehensive understanding of available services and resources for kinship caregivers and provide support in accessing those services.
• Maintain client records.
• Develop and maintain collaborative relationships with other service providers, including participation in monthly meetings with DCF's Regional Gatekeeping Liaison.
• Provide transportation support where needed.
• Be available for on-call duties.
• Perform any other department or agency-related duties or special projects as directed by supervisor.
