About The Position

Responsible professional work delivering family support services utilizing a broad range of community services and natural support systems. Involves a broad range of training and development activities, assessment services, and consultations and requires quality documentation of services provided. Participates in organization’s performance and quality improvement efforts.

Requirements

  • Bachelor’s Degree from an accredited university or college in Human Services field
  • One year of experience working with children and families
  • Possess and maintain a valid Florida driver’s license with no record of criminal driving offense of license suspension
  • Possess means of providing job related transportation and show proof of required insurance
  • Insurable under CHN’s current auto insurance policy
  • Ability to work flexible hours, including evenings and weekends
  • Successfully complete and maintain FDLE clearance, Federal background and State criminal background check and sexual predator screening
  • Current CPR and First Aid Certification
  • Ability to communicate effectively, verbally and in writing
  • Ability to establish effective working relationships with clients, co-workers, supervisors, and other individuals
  • Possess effective interpersonal skills
  • Knowledge of agency’s organizational structure, standard operating procedures, and policies
  • Knowledge of Department of Children and Families rules and regulations regarding the care of children
  • Knowledge of child abuse or neglect reporting procedures and methods
  • Knowledge of intervention and behavior management methods, strategies, and techniques
  • Knowledge of quality documentation as required by agency standards, rules, and regulations
  • Ability to conduct assessments to develop family services, and to provide follow-up services
  • Ability to work as a multi-disciplinary team member in a positive productive manner

Responsibilities

  • Maintains a caseload of 12-15 families and not to exceed 18.
  • Maintains weekly contact with all families on active caseload by means of home visits, office visits, phone contact or correspondence.
  • Facilitates support group for relative caregivers regularly.
  • Coordinates with community partners to provide information and training to caregivers.
  • Ensures families who complete the program will accomplish Support Plan goals.
  • Ensures that children living with relative caregivers will remain outside of foster care one-year post discharge from the program, in accordance with Kinship program expected outcomes.
  • Creates, implements, and evaluates effectiveness of each family’s Support Plan.
  • Completes Assessments, program statistics, and other required documents in the specified amount of time.
  • Ensures that families have follow-up linkages into the community once completed program in accordance with Kinship program expected outcomes.
  • Family Support Coordinator follows-up on all referrals to determine if family received services.
  • Assists families navigate community resources and addresses barriers, including transportation, advocacy, legal, medical, and mental health needs.
  • Meets with Program Manager on a weekly basis for supervision to review cases, training, and professional development and program development.
  • Works within the philosophy and mission of the Children’s Home Network.
  • Participates in quality assurance reviews of the program services, outcomes, and records.
  • Performs other related duties as assigned.
