About The Position

The Program Coordinator, Navigation Services is responsible for the coordination of Integrated Harm Reduction medical services operations and direct supervision of staff and peers. Under the supervision of the Senior Director of Medical Services, the Program Coordinator is responsible for maintaining the department’s readiness for serving clients through our mobile and in-house medical services. Hours for this position are Monday – Friday, 8:00 AM–4:00 PM or 9:00 AM–5:00 PM. Mobile medical services are conducted utilizing a harm reduction approach that supports the client in identifying their own goals around medical care, substance use, sexual health, and engagement in additional services. This position supports the efforts of the Senior Director, AVP, and Senior Clinical Director to strengthen staff members’ clinical skills around client engagement and motivational interviewing.

Requirements

  • A minimum of a Bachelor’s degree or commensurate work experience of three years working in community health care, harm reduction, HIV prevention programming or HIV/STI testing is strongly preferred.

  • Strong commitment to harm reduction.
  • Knowledgeable about HIV infection, treatment, risk reduction; knowledgeable in local, state, and federal laws regarding HIV and other relevant issues of providing health care, especially the right to privacy and confidentiality.
  • Knowledgeable about HCV infection and treatment.
  • Knowledgeable about substance use issues, including but not limited to opioid epidemic, overdose prevention, safer injection practices, Buprenorphine/Suboxone; knowledgeable in local, state, and federal laws regarding Buprenorphine and other relevant issues of providing health care.
  • Strong knowledge and access to New York City social services system, especially in upper Manhattan and the Bronx.
  • Excellent verbal and written communication skills.

Nice To Haves

  • Familiarity with online data entry systems such as ECW, AIRS, eSHARE, eIcare preferred.
  • Bilingual English/Spanish or English/French is preferred.

Responsibilities

  • Coordinate street-based service delivery in the field to ensure proper client flow and assessment of needs during outreach hours.
  • Conduct assessments with clients who use drugs and assess for medical services and other health related services.
  • Assist navigator with connection to care activities with clients, as needed, including medical, insurance, case management, mental health, housing, treatment adherence, and other supportive services.
  • Ensure access to program by providing targeted outreach and education to members of the community and service providers through presentations and community networking activities.
  • Identify resources and promote program; serve as liaison to internal and external service providers and the community by conducting presentations on all program activities.
  • Ensure the maintenance of appropriate, accurate, and clear documentation of all outreach, education, and client activities, including chart documentation and progress notes.
  • Assist in the guidance of the program’s peer educators, providing assistance for their work with clients in the field.
  • Manage timely submission of documentation and data entry from staff and peers.
  • Assist Senior Director in gathering information for monthly reporting as indicated for assigned contracts.
  • Attend contract meetings with funders and any other required community meetings as needed.
  • Work with Senior Director to ensure coverage for services as needed when staff are out of office on PTO or sick time.
  • Assist Senior Director in tracking inventory and ordering needed supplies.
  • Perform other duties as assigned.
  • Recruit, hire (with guidance from supervisor) and train new staff members when needed.
  • Evaluate staff monthly by conducting Data Driven Supervision sessions.
  • Facilitate 90 Day and Annual Evaluations of staff.
  • Conduct observations of staff engagement with clients.
  • Regularly identify and celebrate staff strengths.
  • Regularly identify areas for staff improvement and work with them to develop tools to support their professional growth and the demands of the program.
  • With the guidance of the Senior Director, utilize progressive discipline when the efforts above are not met with skill enhancement or behavior change from staff, or when there is a violation of the agency’s policies and procedures.
  • Enhance and maintain morale of team.
  • Review and approve staff PTO and sick time and complete biweekly timesheet approval in ADP.
  • Supervise Peer Educators assigned to work with the mobile medical services team.
  • Work with Peer Coordinator and Senior Director to ensure agency-wide peer policies are being communicated and followed by department’s peers.
  • Participate in monthly group meetings/trainings as part of peer supervision in order to find challenges and successes when providing outreach services.
  • Support interns and volunteers during their hands-on training experience.
  • Create monthly schedule for peers.
  • Track and submit peer timesheets for payment.
