About The Position

The Program Assistant works in the Adult Day Health Program at the Stoughton Adult Day Health Center, providing care, activities, and support to older adults and individuals with rehabilitation needs, dementia, or intellectual and developmental disabilities in a community-based setting. The role supports participants’ care, independence, safety, and quality of life through person-centered assistance and engagement.

This is a part-time position for 24 hours per week, Monday through Wednesday, from 8:00 AM to 4:00 PM. No evening or on-call shifts are required. The stated wage is $16.00 per hour.

Requirements

• Be at least 18 years old.
• Have a high school diploma or equivalent.
• Have at least one year of experience supporting seniors or adults with medical or rehabilitative needs.
• Have knowledge of vital signs, weights, and activities of daily living (ADLs).
• Be able to pass background checks, a physical exam, and TB screening.
• Be compassionate, dependable, and a strong communicator.

Responsibilities

• Provide daily care and support to older adults and individuals with rehabilitation needs, dementia, or intellectual and developmental disabilities.
• Assist with activities of daily living (ADLs), brief changes, transfers, and personal care.
• Support activities, social skills, and participant independence.
• Lead musical and recreational activities.
• Ensure participant safety and well-being.
