Provides direct support within a program or unit, typically serving 4–6 individuals in residential settings or up to 10 individuals in periodic programs. Support is delivered according to individual service or program plans and may include socialization, health maintenance, medication administration, and skill and behavioral development. The role also assists individuals with personal care, housekeeping, recreation, transportation, community integration, shopping, citizenship, and other activities of daily living. The caregiver documents progress and activities, reviews records and logs for service-plan changes, maintains confidentiality, respects individual rights, and reports suspected abuse, neglect, medical events, behavioral incidents, and other incidents as required. The role may include accompanying individuals to medical appointments, administering or supporting self-administration of medication when assigned, monitoring health, preparing meals according to menus and special diets, and using medical equipment according to procedures. The position works scheduled hours, attends required orientation, ongoing training, and monthly staff meetings, and may cover other shifts when needed. It may also involve transporting individuals, maintaining a clean and safe environment, performing minor repairs or yard maintenance, participating in safety drills, and helping protect individuals during emergencies. The role has no supervisory responsibilities and is based at the program location, with possible travel into the community. For New Jersey positions, employees must cooperate with inspections and investigations; hold a valid driver’s license in good standing for their state of residence; complete required drug testing, background checks, registry checks, and training; and be cleared by the Department of Human Services before working with individuals served.
