About The Position

One Medical is seeking Primary Care Physicians to join its growing outpatient Senior Health practices in Charlotte, North Carolina. One Medical Seniors provides relationship-based primary care, with a focus on adults on Medicare and patients who may need additional attention. Its care model includes smaller panel sizes, team-based care, outcome-focused payment, and technology-supported practice innovation. Team Physicians work closely with Medical Directors and care teams to deliver comprehensive primary care and contribute to ongoing practice innovation. Physicians use One Medical Seniors coaching and integrated behavioral health models to engage geriatric patients and support behavior change, and use the proprietary electronic health record to plan individual care and improve population health. The role is based at the Pineville Office in Charlotte. The position also requires willingness to obtain additional state licensure and credentialing for One Medical virtual primary care in additional states.

Requirements

At least 2 years of primary care experience.
Currently licensed in North Carolina or able to obtain North Carolina licensure.
Board certification in Internal Medicine or Family Medicine.
Ability to build successful relationships with team members and communicate effectively one-on-one and in groups.
Ability to thrive in a fast-growing, mission-driven organization focused on using data to improve patient outcomes.

Responsibilities

Provide comprehensive primary care in an outpatient setting.
Partner with the care team and use coaching and integrated behavioral health models to engage geriatric patients in care and support behavior change.
Use the proprietary Electronic Health Record to plan individual patient care and maximize impact on the population.
Work closely with Medical Directors in care delivery and ongoing practice innovation.
Participate in weekly protected provider time for team building and discussion of challenging cases.
Obtain additional state licensure and credentialing for One Medical virtual primary care in additional states, as needed.
