About The Position

One Medical Seniors is seeking Primary Care Physicians to join its growing outpatient practices in Charlotte, North Carolina. The role provides comprehensive primary care to a geriatric patient population in a value-based care environment, using a relationship-based model designed to support adults on Medicare and patients who may need more attention. Physicians work closely with Medical Directors and the care team to deliver care and contribute to ongoing practice innovation. The model features smaller patient panels, no billing or coding, integrated behavioral health and coaching, and technology designed to support care delivery and population health. The role is based at the Pineville Office in Charlotte, North Carolina.

Requirements

• At least 2 years of primary care experience.
• Currently licensed or able to obtain licensure in North Carolina.
• Ability to build successful relationships with team members and communicate effectively one-on-one and in groups.
• Ability to thrive in a fast-growing, mission-driven organization focused on using data to improve patient outcomes.
• Board certification in Internal Medicine or Family Medicine.

Responsibilities

• Provide comprehensive primary care in an outpatient setting.
• Partner with the care team and use coaching and integrated behavioral health models to engage geriatric patients in care and drive behavior change.
• Use One Medical’s proprietary Electronic Health Record to plan individual patient care and maximize population-level impact.
• Participate in weekly protected provider time for team building and discussion of challenging cases.
• Obtain additional state licensure and credentialing for One Medical virtual primary care in additional states, as needed.
• Contribute to ongoing practice innovation in collaboration with Medical Directors.
