About The Position

This role offers physician independence, meaning your clinical decisions stay yours with no conflicting institutional incentives or administrative pressure. The community medicine aspect in Lake Placid focuses on chronic disease management and real prevention within a Medicare Advantage population. The position offers a Monday through Friday schedule with predictable hours. Millennium Physician Group handles administrative burdens through their platform, Millennium CARES Connect, including payer contracting, quality and coding, HCC risk adjustment, credentialing, billing, and population health management. Clinical support is provided by a 24/7 nurse triage line, case managers, hospitalists, transitional care coordinators, social workers, discharge planners, palliative care specialists, clinical pharmacists, and home health. The technology used is AthenaOne EMR with AI integration, designed to reduce documentation time, and includes Virtual Care Clinic and Telehealth services.

Requirements

MD or DO degree with completed residency in Family Medicine or Internal Medicine
Board Certified or Board Eligible
Active, unrestricted state medical license (or eligible to obtain) and valid DEA number
EMR proficiency
Commitment to value-based, patient-centered, outcomes-driven care
