About The Position

ChenMed is a physician-led, mission-driven primary care organization focused on providing affordable, high-quality care to seniors. The Primary Care Physician (PCP) manages a panel of up to 450 patients and is accountable for clinical outcomes, cost-effectiveness, and building trusting relationships that help patients make lasting health behavior changes. The PCP serves as a health coach, helping patients establish and pursue health goals through ongoing follow-up, and simplifies care plans so patients and care teams can act effectively. This is an outcomes-focused, value-based role. PCP performance is regularly evaluated using measures that include patient admissions per thousand, appointment-slot utilization, patient experience, clinical gap closures, and medical costs. The PCP works with center and market leadership and care teams toward established targets and helps foster a positive culture of love, accountability, and passion. The role requires independent practice as well as collaboration with other health professionals, managers, and medical directors to support compliance, quality, risk management, and patient care. The PCP assesses acute and non-acute conditions, performs and documents examinations and histories, interprets clinical information, and develops and implements patient management plans. Duties include ordering and interpreting diagnostic studies, prescribing medications and treatments, making referrals, documenting care, coordinating multidisciplinary treatment, and responding to urgent situations. The role may include invasive procedures when competency has been documented, care transitions and discharge planning, patient and family counseling, and participation in clinical meetings and care conferences. Spanish speaking is preferred.
