About The Position

The Primary Care Physician provides equitable, effective, value-based healthcare to local Medicare patient populations at Oak Street Health neighborhood primary care centers. Physicians work at a single medical center and are supported by large care teams, allowing them to focus on quality of care rather than volume of services. The value-based care model and competitive bonuses are designed to reward outcomes, reduce hospital admissions, promote preventive medicine, and improve patient experience. The role is intended for physicians motivated to provide excellent care to older adults. New graduates are encouraged to apply. Highly qualified candidates may also be considered for a Center Medical Director position.

Requirements

• Graduate of a Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) program.
• Board certification in Internal Medicine or Family Medicine, or board eligibility.
• Active, non-probationary, unrestricted state license.
• Active DEA license.
• U.S. work authorization.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medication.
• Maintain patient electronic medical record data using Canopy and Greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Perform other duties as assigned.
