About The Position

Join the primary care team in Apple Valley, California, providing comprehensive outpatient care for patients across the lifespan. The physician will have clinical autonomy and collaborate with advanced practice providers (APPs) and an interdisciplinary team to manage acute and chronic conditions, improve population health, and deliver high-quality, value-based care. Leadership opportunities may be available for physicians interested in mentoring APPs and supporting clinical quality initiatives while maintaining an active patient panel.

This outpatient-only role follows a Monday–Friday schedule from 8:00 AM to 5:00 PM. CareMore describes its value-based model as focused on outcomes and prevention, with multidisciplinary collaboration and operational and care coordination support. The position offers a base salary range of $253,643 to $380,464, a quarterly incentive plan of up to 15% of quarterly earnings, and eligibility for the Mosaic Health Profit Shares Program. Requirements

• MD or DO degree.
• Active, unrestricted state medical license.
• Board Certified or Board Eligible in Family Medicine or Internal Medicine.
• DEA certification.
• BLS certification.
• Experience in primary care, chronic disease management, value-based care, and population health.

Nice To Haves

• ACLS certification is preferred.
• Interest in mentoring APPs and supporting clinical quality initiatives.

Responsibilities

• Provide comprehensive outpatient primary care for patients across the lifespan.
• Manage acute and chronic conditions.
