Requirements

• Active and unrestricted medical license or willingness to obtain a license in the practicing state OR have an active Area of Critical Need license in Florida.
• At least 3 years of experience in outpatient primary care.

Nice To Haves

• Additional training in Family Medicine, Internal Medicine, or Geriatrics preferred.
• Experience in value-based care.

Responsibilities

• Provide primary care to patients.
• Manage a small patient panel.
• Focus on value-based care.
• Collaborate with a Health Services Team, Regional Physician Coach, Clinical Performance Coach, and Pharmacy Team.
• Utilize administrative, financial, and technological resources.
• Participate in Collaborative Forums.
• Utilize Operations Management Dashboards & Quality Metrics.
• Use Decision Support Tools.
