About The Position

Children’s Hospital of Philadelphia is seeking a part-time Primary Care Nurse for its West Chester Primary Care office. The shift is based on operational needs, and the nurse must be willing to rotate to other Care Network sites within the designated cluster. The Primary Care Nurse is a clinically competent practitioner of pediatric, family-centered nursing. With minimal guidance, the nurse provides safe care to the general unit population and demonstrates competence in caring for patients with moderate to complex health issues. The role follows established standards of care, administrative policies, and clinical protocols. The nurse also assists the Site Manager with administrative issues affecting clinical practice. Nursing-practice matters are administratively accountable to the Site Manager and Ambulatory Nursing Director. The role includes patient assessment and care planning, collaboration with families and care teams, patient and family education, documentation, professional leadership, participation in research and learning activities, and responsible management of time and clinical resources. CHOP requires an annual influenza vaccine for employees working in patient-care buildings or in patient-facing roles.

Requirements

Technical Diploma in Nursing required.
Registered Nurse license in Pennsylvania or New Jersey required upon hire.
Basic Life Support (BLS) certification required upon hire, issued by an organization requiring hands-on instructor-led psychomotor skills verification.
Computer competency, including basic computer use, Microsoft Office, web browsing, intranet searches, document access, and shared network file space.
Ability to use patient-facing technology and clinical communication systems.
Ability to determine clinical information needs and access relevant information effectively and efficiently.
Ability to critically evaluate information and its sources, incorporate it into clinical practice, and evaluate outcomes.
Ability to navigate the electronic health record and locate and review patient data in clinical information systems.
Ability to use clinical decision support tools and understand relevant policies, confidentiality, security, and privacy requirements.
Ability to collect clinical data using quality-improvement tools.
Willingness to rotate to other Care Network sites within the designated cluster.

Nice To Haves

Bachelor’s degree in Nursing preferred.
At least two years of nursing experience preferred.
Pediatric experience preferred.
Previous supervisory experience preferred.

Responsibilities

Use the nursing process—assessment, planning, implementation, and evaluation—to deliver safe care.
Synthesize patient assessment data and recognize complex situations before communicating with the care team.
Collaborate with families, team members, and other caregivers to initiate or update plans of care, education, and discharge needs.
Prioritize and delegate appropriately when managing care for complex patients.
Identify expected outcomes, revise care plans when outcomes are unexpected, and communicate the plan effectively.
Document ongoing evaluation and the effectiveness of patient care in accordance with hospital standards, policies, and procedures.

Listen to families and seek opportunities to improve the patient experience.

Chair or participate in unit PPM council and potentially participate in departmental PPM council.
Read nursing articles, analyze information, and initiate staff discussions.
Assist with research data collection and efforts to apply findings to clinical practice; frame basic research questions.
Identify learning needs, pursue learning opportunities, attend required programs, and apply training to daily work.
Attend education programs relevant to the patient population and use relevant patient-education materials.
Use health-care informatics and clinical information systems to manage patient care and outcome data.
Promote quality, efficient, and cost-effective care.
Monitor supply and equipment use, identify improvement opportunities, and make recommendations.
Manage time efficiently to complete direct and indirect care.
Assist the Site Manager with administrative issues affecting clinical practice and comply with office policies and protocols.
Rotate to other Care Network sites within the designated cluster.
