About The Position
AllCare.ai is seeking Primary Care Nurse Practitioners to provide on-site care to residents of assisted living and RCFE communities. The role offers steady, predictable patient volume and an opportunity to deliver care where seniors live. The position is full-time, with a structured weekday schedule: three days providing on-site care along strategically clustered routes designed to minimize travel, and two days at the company’s Irvine headquarters for administrative and documentation work. Compensation is $140,000–$160,000 annually, based on experience, schedule, and patient volume, with additional earning opportunities based on coverage and panel needs. The role is W-2, with full benefits available and mileage reimbursed per policy. Final terms are outlined in the employment agreement.
Requirements
- Active California Nurse Practitioner and Registered Nurse licenses.
- Active furnishing license.
- NPI.
- DEA registration.
- BLS certification.
- EMR proficiency.

Responsibilities
- Conduct routine and urgent assessments and manage chronic diseases.
- Provide post-discharge follow-ups, medication reconciliation, and labs and diagnostics.
- Coordinate care with facility staff, specialists, families, and powers of attorney.
- Complete same-day documentation and comply with CMS and regulatory requirements.
- Provide on-site care at assisted living and RCFE partner communities.
- Complete administrative and documentation tasks at the Irvine headquarters.
