About The Position
The Nurse Practitioner provides continuing and holistic healthcare services to individuals and families. The role delivers preventive, chronic, and acute care, including diagnosing and treating illnesses, conducting health-risk assessments, routine check-ups, screening tests, and immunizations, and counseling patients on maintaining a healthy lifestyle and preventing disease. The Nurse Practitioner follows current evidence-based guidelines and procedures, participates in continuing education and training, and takes part in quality assurance audits.
Requirements
- Nurse Practitioner license in the State of Illinois.
- Current Illinois Controlled Substance License within six months of hire.
- Eligible for DEA registration and possession of a current DEA Registration within six months of hire.
- Current ACLS and BLS certifications.

- Bilingual in Spanish strongly preferred.
Responsibilities
- Deliver preventive, chronic, and acute healthcare services.

- Participate in continuing education and training to remain current with developments in the industry.
