About The Position

VenVisor Health is hiring a Primary Care Nurse Practitioner/Physician’s Assistant to provide on-site care to residents in skilled nursing and long-term care facilities. This full-time position is intended for a provider who values meaningful patient relationships and collaborative care. Requirements

• Hold an active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• Hold a state-specific license in good standing in North Carolina.
• Have at least 6 months of experience in long-term care or a skilled nursing facility.
• Demonstrate strong communication, clinical, and critical-thinking skills.
• Have a passion for elder care and person-centered healthcare.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical examinations and assess medical histories.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and laboratory results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and care goals.
• Help ensure a safe, respectful, and supportive environment for residents.
