Primary Care Nurse Practitioner or Physician Assistant
About The Position
Bloom Healthcare is hiring a Primary Care Nurse Practitioner or Physician Assistant to provide relationship-based primary care to homebound seniors and medically complex patients in their homes and communities in the Lakewood, Colorado territory. The role offers meaningful clinical autonomy, manageable patient volumes, and support from an interdisciplinary team and advanced data systems. The provider serves a defined patient panel, responds to acute needs, and works to improve health outcomes while helping patients age safely and comfortably in place. The position follows a Monday–Friday schedule, with 8–12 patients per day, weekends off, flexible daily workflows, and a light call rotation of only 2–3 times per year. Compensation is $100,000–$125,000 annually depending on experience, with potential performance-based bonuses and benefits. Benefits begin on the first day of employment.
Requirements
- Nurse Practitioner or Physician Assistant with a master’s degree.
- Active Colorado licensure.

Responsibilities
- Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patients’ homes and communities.
- Diagnose, manage, and treat chronic and acute conditions with support from clinical operations, the triage team, and MD leadership.
- Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers.
- Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments.
- Complete comprehensive evaluations, including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.
- Use Bloom’s integrated data ecosystem to track hospitalizations, coordinate post-discharge visits within 72 hours, and improve outcomes.
- Collaborate with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.
