Primary Care Functional Medicine - Nurse Practitioner
About The Position
OnePeak Medical is seeking a passionate and skilled medical provider to deliver whole-person, root-cause-focused primary care in a supportive, forward-thinking environment. The provider will deliver comprehensive medical services, build lasting patient relationships, and collaborate with a high-functioning care team to support optimal health outcomes. Care is provided through office visits, telehealth, and occasional same-day appointments, with personalized plans that may incorporate lifestyle interventions, supplements, hormone therapies, and prescriptions. OnePeak offers functional medicine onboarding, clinical mentorship, and ongoing training, along with administrative, coding, and billing support and access to medical assistant and medical scribe support. The position offers a guaranteed salary with earning potential of $300,000 or more, relocation support, and a four-day work week with a flexible schedule and no nights or weekends.
Requirements
- Active Oregon license as a Nurse Practitioner, PA-C, MD, or DO, or eligibility to obtain one.
- Board certification in Family Medicine, Internal Medicine, or an applicable specialty.
- Strong interest in preventive, functional, and lifestyle medicine.
- Excellent communication and patient relationship skills.
- Comfort working in a fast-paced, collaborative environment.
- Proficiency using electronic health records.
- Accepted Nurse Practitioner licenses include FNP (Family Nurse Practitioner) and AGNP-PC (Adult Gerontological Nurse Practitioner—Primary Care).
Responsibilities
- Provide direct patient care through office visits, telehealth, and occasional same-day appointments.
- Perform comprehensive assessments, order and interpret labs and imaging, and diagnose acute and chronic conditions.
- Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions.
- Educate and empower patients on preventive health and functional medicine principles.
- Collaborate with health coaches, nutritionists, mental health providers, and clinical support staff.
- Document visits in the electronic health record in a timely and compliant manner.
- Participate in team meetings, quality improvement initiatives, and training sessions.
- Support clinic operations by contributing to a positive, team-oriented environment.
