About The Position

The Population Health RN assists with the collection, monitoring, and reporting of population health and quality data for participation in governmental and commercial insurance quality programs. The role serves as a liaison between physician services and the quality department to help ensure Margaret Mary Health practices meet quality and population health goals. The position is located in the Medical Arts Center, is scheduled for 80 hours per pay period, and is benefit eligible. There are no supervisory responsibilities.

Requirements

Associate degree in nursing at minimum; a bachelor’s degree is preferred.
At least one year of relevant work experience.
Current Registered Nurse license in the state of Indiana.
Basic nursing skills.
Exceptional interpersonal and telephone skills.
Good organizational skills.
Effective verbal and written communication.
Ability to operate general office equipment and patient care equipment as needed.

Nice To Haves

Bachelor’s degree in nursing.

Responsibilities

Use only approved abbreviations.
Understand patient safety goals and actions taken.
Use two patient identifiers when necessary.
Report sentinel events or incidents as warranted.
Maintain working knowledge of quality programs in which Margaret Mary Health participates.
Monitor practice quality metrics.
Assist practices with collection of required quality and population health data.
Report population health data in the required format.
Serve as liaison with practices, third-party payors, and the quality department.
Educate providers and staff about documentation needed to meet quality measures.
Provide feedback on current processes and procedures to meet and improve population health goals.
Attend Care Coordinator, Quality, and ACO meetings.
Coordinate patient outreach efforts where needed to meet population health goals.
